Patient Choice creates and
manages patient-focused health care networks. The company partners with a
number of administrators to do claims processing and customer service
for its care system programs. Patient Choice Select is administrated by UMR (formerly Fiserv Health).
If you have questions about your plan, including medical eligibility, benefits or claims,
you should contact UMR at 1-877-559-4833, or go to the UMR Web site
at www.fiservhealthservices.com and select “Members.”
If you are looking to enroll in Patient Choice and have not received a medical
identification card, check your employer's enrollment material for the
appropriate contact information.
